Sanford Center for Aging, a dedicated organization serving the needs of older adults, received $783,881 in grant funding for the upcoming fiscal year 2025 from the State of Nevada Aging and Disability Services Division and the Department of Public and Behavioral Health. This generous funding will significantly bolster the organization's efforts to provide essential programs and services that enhance the quality of life for older adults in the community.
The newly acquired funds will be allocated to a variety of free programs, including:
- Community-Based Health and Wellness: This free service includes blood pressure checks, wellness screenings and education.
- Diabetes Management and Prevention: Offering education to persons living with diabetes and those at risk of the disease.
- Healthy Steps for Older Adults: The evidence-based workshop reduces falls while providing participants with steps to make homes safer, manage medicines, improve overall fitness and more.
- Heart Disease and Stroke Prevention: Provide the Healthy Heart Ambassador program to older adults at risk of developing cardiovascular disease; and deliver the Fit and Strong program to people living with hypertension and high cholesterol.
- Medication Therapy Management (MTM): A geriatric pharmacist will review all medications and supplements to identify unsafe combinations, improve understanding of both medications and supplements while also discovering which medications can increase falls or memory loss.
- Senior Outreach Services (SOS): A companionship program, SOS matches volunteers with isolated older adults in the community to build connections and support.
- Social Work Assessments: Provided by Sanford Center social workers, these assessments identify social and mental health issues with individuals and provide resources and support.
- Transportation Services: Increasing availability and accessibility of transportation to help older adults maintain independence and participate in community activities.
